As a result of our church mergers, the databases of Assumption, Sacred Heart, and St. John Neumann have been combined and it’s clear that many phone numbers, emails, and addresses are out of date. So, under the guidance of our pastoral council, we're launching a Parish Registry Update.
All registered households will be mailed a letter with a Connection Card asking for updated contact info. We ask you to kindly take just a couple minutes to review, fill out the form & mail in the enclosed envelope, drop it into the church collection basket or return it to the church office. One person from each household can also follow the QR code on the form (or the link below) and update the information online. It's helpful if you have your envelope number (included on the Connection Card - but not required) if you choose to use a QR Code or link below.Click here to fill out the electronic form
